New Patient Appointment Scheduling: To schedule a new patient medical, dental or vision appointment, please call Central Registration at 616.965.8308 or email email@example.com. To schedule a counseling appointment, please call one of our counseling centers directly.
Cancel, reschedule or schedule an appointment: To cancel or reschedule an appointment, please call your health center or use the Patient Portal. We cannot schedule, cancel or reschedule an appointment through our Contact page or through email.
Online Bill Pay: We do offer online bill pay. Please click here to pay your bill.
Patient Portal:To get signed up for Patient Portal, either call or stop into your health center and ask for an enrollment token. We cannot issue enrollment tokens via our Contact page. Please call your Health Center with any Patient Portal questions.
Request Medical Records: Please visit our website here to learn how to request any health record (including medical, dental, vision, behavioral health, vaccination, etc.) from Cherry Health.
Prescription Refills: To request a prescription refill, please call your pharmacy and request the refill, or visit your Patient Portal.
Provider Communication: If you need to speak to your provider, please call the office where you receive care. Messages sent via the Contact page will not be passed onto any healthcare provider.
