Durham Clinic

The Durham Clinic is a fully integrated health care clinic with a team-based approach to the management of chronic physical and/or behavioral health conditions. The goal is to help patients manage their health conditions well enough so that those conditions do not interfere with how they want to lead their lives.

The team members include: internists, psychiatrist, nurses, health coaches (Master’s Level Social Workers), supports coordinator, peer support specialist, medical assistants and a physician assistant.

Team up for better health! Patients with more than 1 chronic health condition (such as diabetes, high blood pressure, etc.) may be eligible to receive support through a program called MI Care Team. This team of health care professionals works with patients to reduce barriers and improve their health and wellness. Interested? Ask about it at your next visit!

Eligibility: Any individual who is 18 years or older and has at least one chronic health condition (mental or physical) is eligible for services. If you have questions or would like to receive Durham services, please contact the Durham Clinic at 616.965.8283.
